I need to know the boxes i need to fill out on my w-9

You need to fill in your name, address and social security number. Indicate whether you are single or married. If you are married, you can also elect "married, but withhold at single rate". Now put down how many exemptions you are claiming. Use the worksheet at the top part of the page if necessary. The more exemptions you claim, the lower the amount of taxes withheld from your check. You can also elect to have an additional flat amount withheld along with the calculated amount.

Just fill everything out up to the signature line. The box you check on the line under "Business Name" depends on how you are set up. You probably will not check the "exempt from backup withholding" box.

I'm not sure exactly what you're having problems with?
If you're a Sole proprietor, list your name, your business name (if you have one, including Doing Business As/DBA), check Individual/Sole proprietor, list your address, city, state and zip code.
Taxpayer ID can be your social security number or a Federal Employer ID Number(if you have one).
Listing account numbers and the requester's name & address is optional.
