Go to usps.com and locate a post office that has a passport office. There you will find the forms you need to fill out to apply for a passport. Those forms will also indicate what supporting documentation you need to provide to prove that you're a legal citizen of the U.S. ... which you should be able to do, *if* you're a native from Alabama (and you haven't surrendered your citizenship).
