Yes, they should be sweated. This means you cook them in butter until they are clear but not carmalized. You don't necessarily have to, they will still cook with the rest of the meatloaf, however, they will have a nicer taste if you sweat them first.

In most cases no, you do not cook the onions before putting them in the meatloaf but there are exceptions to every rule. The onions will fully cook inside the meat as the meat cooks. It is not unheard of though to precook the onions, say because you carmelized them or cooked them in red wine to add that flavor or cooked them a special way, that will all add flavor to the meatloaf. Then you can still add them to the meat loaf.

Oh, one more thing. If you do precook them let them cool before adding them to the meat.
If you are making individual meat loaves then you may want to cook them ahead of time because individual meat loaves can cook in thirty minutes. The onions would be a little firmer but still good if placed in raw but by precooking them you get that soft and sweet taste.

When I make meatloaf, I generally put the uncooked onions in with my meatloaf and everything will cook up together. But everyone is different. Try cooking your onions first and then put them in your meatloaf and then the next time you make meatloaf don't cook the onions and see which version is best for you.

no you don't cook the onions b 4 you put them in your meatloaf mixture. to make really good meat loaf season the meat first then add onions and uncooked oatmeal then cook with a little water.

No, they will cook as the meatloaf cooks. If you precook them they will most likely release much of the flavor during the first cooking and likely be mush in the meatloaf.

I sautee them with garlic and then add them to the meat. No one in my family "loves" onions, but it helps give the meatloaf flavor and they absorb some of the garlic.
