Being female I shall tell you what she wants/ed for dinner. Not what you suggested/.made, whatever it was. You should have known what she wanted without having to ask(!)
Honestly, does she just want you to do dinner for a change because she normally does it? If that is the case, cheese on toast will do fine!

Do the Hamburger Helper thing , it's very easy. You can also get the frozen veggies to go with , you just throw them in a pot and turn the stove on and that takes care of itself.

Ask her what she hates and don't get that. Anything else will work. If your in to experimenting I would suggest sushi. It is a good meal to share with someone. Just make sure you go to a good place (they should have sake).

She can't make up her mind, or she's apathetic?
If she always replies with, "I don't know," or, "Doesn't matter to me," then choose where you want to go, as long as you think she will like it too (don't take her to your favorite sports bar when you know she'd rather go to a quiet restaurant.)
But if she just can't decide, but you know she'd like to pick, then tell her where you want to go. If she doesn't like your picks then she will step it up and pick somewhere she likes. Nothing helps you make up your mind like someone suggesting places you don't like!
