That sounds a little odd. If she did say this I hope she was joking, even so, it's very unprofessional. As a pastors wife, I doubt that she meant what you were thinking. However, I've heard of worse. She may simply have meant for some relaxing back rubs, and didn't realize how it sounded until after she said it. We've all had embarrassing moments like that. As far as doing massage as a fundraiser that's fine.

No, you should not say a word to her nor her husband. If you needed clarification, you should have asked her meaning when she offered a "happy ending". You can quit obsessing over her meaning. Going on about it only makes you come across as meanspirited and vindictive. Do you know for sure that her "happy ending" isn't sharing a personal witness to her faith in her god? If the thought makes you uncomfortable, just don't go for one of her massages again.
Certainly, if she's a licensed therapist, her using those skills as a fundraiser for her church is perfectly acceptable. Several of my colleagues - professional nurses - regularly provide their services as fundraisers. It is legal and acceptable. So don't sit there blithely insinuating evil intent: the most evil I detect here is your dirty mind!

Your story stinks of inventive fiction - however on the very slim chance you're being honest (still doubt you are)...,
I would be leaving that Church in a heartbeat. That sort of stuff should not be going on at all - especially in the Church building itself. For a pastor's wife to be doing such things in the Pastor's study - he would have to be fully aware of what's going on.
Get the heck outta Dodge - uhm..., that particular Church too.

Yes, by all means, ask her in front of her husband, so the air can be cleared. Don't be accusatory, just ask what she meant because that confused you and stuck in your head.
It may have been that she didn't want to make you sore afterwards .... I remember our office treated all of us to back massages in those weird chairs by licensed therapists, and we all walked away sore for days lol. That's one deep massage!
