Just say, like :"Hey this is your former student (your name) and I just wanted to stop by tomorrow, is that alright with you"?

Writing an e-mail is so cold. I think you should phone her, it shows more consideration. Anyways, it's up to you whether you take the class or not, so I think that it's just politeness calling her so you should be like "I'm calling you to let you know I'm not assisting to your classes any longer" rather than "Is it OK if I stop..."
Good Luck!

You're doing the right thing by not just "Sliding in". It could be very disruptive. Maybe call the office and find out what they reccomend, and when might be a good day. . .for BOTH you and the teacher in question.

Good job!
