Nah, maybe he just feels more confident in a hat. You could always say something like, "I've seen you so much, yet I have no idea what is under that mysterious hat! Seriously, what's under there?" Haha, I'd say keep it fun with this guy, and if he is bald, dont make a deal, because maybe he's super self conscious.

Not necessarily. For some guys wearing a hat is the same as wearing pants. They would feel naked without it. I once worked with a fellow who always wore a ball cap. One day when a bunch of us came in to work, we were wondering who the new guy was. It was old ball cap without one. He had a full head of hair. Just didn't like to go without a cap.

He's either embarrassed about his hair (doesn't have to be bald) or doesn't respect you. Not taking your hat off in company (especially indoors) is a sign of disrespect.
