I have a grandma that gives me $20 secretly everytime she sees me. I know she means well, but I really feel bad taking it from her. Is there a polite way to discuss this with her?

No. Take it and thank her. Her generation feels the need to provide for the younger generations. It helps her feel as though she is contributing to your life. It's her way of showing you that you are important.
Then take all that money put it aside to help her with a purchase down the road - whether it's a car or repairs; health care costs that insurance won't cover, like a hearing aid or dental work; etc.
This way you won't have insulted her by refusing her gifts, and you will show her what a nice young man you've become. It's a win-win!

It probably makes her feel good. Let her continue. If she needs the money herself, buy her food, clothes or pay part of her power bill with the $20.

I have a grandmother like this. I always just put it back on her table or in her coat pocket so that she will not be offended that I did not take it. She will just find it and think she forgot about having it.

Just tell her you don't want it. If she keeps giving it to you, then just stick it back in her purse when she's not looking.

Edit: no not for sexual abuse, i mean she would take anything even if it was the most well, "nasty" thing for me.
