Wire gauge is selected based on current capacity and voltage drop. Insulation type is based on voltage, environment, and type of service.

There are published standards for many different applications. Safety factors are figured into the requirements.

By what punishment they have to take from the weather and the kind of current we want to put through them. Just read the NEC.
