My goodness, having met people who liked scat (feces), who liked to wear high heels and be dusted in baby powder before they could have sex, people who liked soiled underwear, really dirty underwear that had been worn for a month, people turned on by body odors and partners who didn't bathe for a month, watersports, rainbow showers (being vomited on during sex), etc, etc, etc, in the course of my research, I would have to say "it'd take me a while to decide what I thought was the most bizarre".
Off the top of my head, though, I knew a gentleman who liked to have someone cut holes in him and then have sex with the hole (no, not kidding... adored being stabbed through the thigh and having someone do the deed in the wound). That's the most bizarre one, and one I reacted to with the most alarm, that I can think of right at the moment.
P.S.-- Let me just say right now you shouldn't take the fact I have met people who liked those things as an indication I am fond of practicing any of them. I'm just REALLY non-judgmental about the ways in which other people like to be consentingly kinky and freaky with each other.

The most bizarre would be one that you cant have any concept of understanding why anyone would want to do it. Like someone paying to defecate on their face. Or likes to be repeatedly kicked or stomped on (groin). for example? to many to list.
