We dated for 3 months already and she needed a time to herself without a boyfriend, i supposed to ask her again once I feel our break has been long enough, which has been over 6 months, but i don't know how to approach her with it, help??

you already waited too long. i don't think she meant 6 months. plus, you don't have to ask her out. just call her and say hi. see if she wants to hang out sometime. tell her how you feel.

Why dont you ask her out on a date together with some of your closes friends. At least at that point, you are on a date and that you can still talk to her normally. Just be yourself ok? If you dont like the group date, then go straight and ask her out personally (dnot on the phone). Asking her wouldn't do you any harm, at least you tried right? But if ever you did asked her and she refused, then give her more time to think things over. You see, girls are sometimes confused, so kindly bare with her. Just be yourself, give her time to think and be honest with what you feel towards her..

walk up to her and start a normal conversation. see if maybe she wants to do something in the open in public wear you cant do anything and there will be no pressure. then after that for a couple of "dates" and after you get her number call her and see if she wants to do something more personal!
