I have been once. It was better than a couple of good shots of Jack Daniel's! I wasn't fully out, though. The room was more spinning around me as I slowly sank to the floor.
As for how it happens: yes, well trained fighters usually do develop good timing and reflexes, but that doesn't mean that the other fighter can't have them too. Watch any fight (in any discipline): unless it is a great mis-match, both people WILL get hit. Getting knocked out depends on two things: the force of the blow and where it lands. There are a few spots on the head that can lead to easy knockouts and if a blow lands there (often by sheer luck), out you go. The main ones are the temple (and surrounding areas by the eye), the occiput (the softer part at the base of your skull) and the "button" on the chin. Hitting the temple and the occiput will jostle the brain and can easily lead to a concussion. The there is a big nerve that runs from the jaw to the brain that can black you out from its disruption if it gets hit. You can also get knocked out from getting hit anywhere on the head if the force is sufficient to cause a concussion (making the brain bash up against the inside of the skull, giving it a nice bruise).
To demonstrate the effect on the button (the safest of knockouts--if there is such a thing), try this: clench your jaw. Place your hand so that the webbing between your thumb and forefinger is on the front of your chin and your thumb is on the side, along the jawline, below the teeth. If using your right hand, your thumb should be on the right side of your face. Remember the spot where the first knuckle of your thumb lies (the first knuckle down from the tip). With your knuckles, give yourself a light but firm rap on that spot, coming down on an angle toward the jaw. Your head should feel a little funny.
Enjoy! But remember, never, NEVER do this more than lightly enough to feel that, yes, you could get knocked out if someone hit you there hard enough. A knockout is never good, mmmkay?

Yeah have been a few times, large flying stone to the temple. Couple of windshields. Maybe the other person has faster reflex's and after you get beat in the head a hundred or so times it kinda slows you down.

I have been knocked out twice in over 50 fights. The 1st time was from a sucker punch right on the jaw. The guy was 6'4" 250 lbs (My older brother). I did not even feel that one. I woke up and my jaw did not even hurt. I guess it was a very clean punch. The second one was while in the army. I got into it with a huge power lifter. He caught me right between the eyes. (7 stitches in my head and 2 black eyes). He destroyed his right hand and was on his 2nd surgery when I was sent to Korea. My black eyes and stitches were gone in 10 days. Who won? Never hit a samoan in the head.

Focus plays a role on it however usually you make a mistake and miss a block. Last time I was knocked out it was in the middle of a sparing match with my teacher and he threw a fast punch then landed a snap kick to my temple and the world went a spinning.

Not so far in TKD. I did walk around the corner of the Industrial Arts Bldg. in Jr. High jst as this kid was following through on his swing and the bat caught me flush across the right eye and bridge of my nose. Broke my nose, and thought I was going to lose my eye, but eventually it got better. Deviated my septum and soaked my pristine white gym clothes in bloody red. I guess I was quite a sight. I don't remember any of it until I woke up with the doctor pushing his forceps up my nostrils to straighten out my snout.

I haven't been knocked out yet, but it usually happens by the blow you didn't see coming. Once the brain-stem is twisted far enough, whether you saw it or not...lights out.
