If it just came off you need to determine why. It's most likely the belt is worn or your tensioner pulley needs to be replaced.

You must fix what caused the belt to come off.
After doing so, follow instructions here to install a new belt

If you have a 6 cylinder:

If the belt came off then you probably have another problem causing it. Start by getting a new belt and checking all the pulleys are tight (you shouldn't be able to wobble them.
