Wow, I have been in America for 36 years and I have never heard that one. But, I live in New York and different parts of the US have different slang.

... I know a "sixer" is 6-pack of beer, so perhaps it means a 12-pack of beer. Or maybe a 12 means a 12 inch sandwich.

maybe you mean "at 12" as in at 12 o'clock, that would be directly in front of you and is more old fighter jock slang then American slang.
