I would discuss the improved customer relations and sales as a result of my networking with our clients outside of work hours.

Woo hoo, what a mine field!

You need to be deviously honest. First, the honest answer to your own perspective shows your values, or lack of values, for the enterprise. You still need to be prepared for answering explicitly that if cornered, but you also need to be answering to the asker's perspective and values. Ask yourself, what does the asker perceive as important, and then review what you have done that fits on that list.

A child asked his mother, "Where did I come from?" The mother hesitated, the time for "That" has arrived, "When a mother and father love each other very much, they do things that start a family. This starts inside the mother, she grows the child inside of her until it is ready, then the mother goes to the hospital where they help her help the baby out while staying ready in case something goes wrong." The child looked bored, "Mom, Billy said he came from Tulsa, where did I come from?" If you know where the asker is "coming from" you can tailor your question. Otherwise you may be saying things that either don't matter or go places you really didn't need to go. I hope you've made notes on your job that can help you figure out what needs to be said, otherwise it would be good to recreate that before you go to the review.

well, that's an individual answer....

my biggest accomplishment since my last performance eval was leaving the company!!! it was horrible and i was very proud of myself for sticking it out. i gave them a miserable year of my life. i agreed to stay at least a year and 2 months before the year i told my boss that i'd be looking for a new job come first of the year. is stuck it out and he told me the day i left that if i ever needed a reference he would be happy to give me one. he told me that he didn't know if he would have even stuck it out as long as i did.

so, that was my accomplishment...what's yours???

did you receive commendation for something?
have you consistently hit your goals or exceeded them since your last eval??
