I dont know about all of them. But some hotels ive stayed in, they come to your room in the morning and take all the blankets, sheets and all the covers for wash and put in new ones. This was a 4 star hotel. Was only 4 star because it had 1 flaw which was that it was thatched roofed which is easier to catch fire.

Most hotels only change and wash the sheets, not the bedspreads or blankets. If you are unsure, you can always ask them to put fresh blankets on your bed.

Some do and some don't. Best advice, always carry a small "blacklight" with you when you travel. Shining a blacklight on the blanket will make it obvious if they do or don't, as any stains will show right up.
