It's all about risk assessment in my opinion. You need to decide what would cause the most negative impact; telling the truth or keeping quiet. If you feel that telling them would be for the best you need to stick by your decision 100%. If they do get hurt by the news then atleast you can take comfort in the knowledge that you felt you done what was right.
On the otherhand, by not telling them you must realise that you have not done so for a reason. Either way, the very nature in which you have asked this question indicates you are a fairly thoughtful friend whatever your decision.

YES no doubt about it you have to give an honest answer or you would be a liar or at worst a fraudulent cheating nobody that would not know the truth if it bit them on the donkeys behind not that I'd want to disappoint or offend anyone with this answer :)

Yes and then again No! generally when you get asked this question an honest answer leads to trouble because your friend already knows what the answer is anyway! They just want to avoid the consequences and dump on you.
Solution is to turn the question back to them by using what is called open questions.
Example. Q. Does my bum look big in this? (Jeans)
Answer, You always look great anyway. How do you think it looks?
Works for allquestions; serious or not!

I think you should be honest but be nice about it. And if they get upset with the answer remind them that they asked you for an honest answer. Just remember that when telling the answer just be as kind as possible about it and maybe it won't upset them as bad.

When my friends ask me anything I always answer them honestly. If I know it will upset them I usually say something like, Do you really want me to honestly answer that because I think it will upset you.
Or if its something simple like, do I look fat? If they do I say well I dont think that look is very flattering on you why dont you wear . . .Me and my friends have great relationships because we are honest with each other, it extends the relationship beyond the superficiality that most friendships are.

Yes,if they want an "honest" answer, I always give them one. I try to be sensitive to them, and use tact, but if they ask me, I'm going to tell them the truth., even if it upsets them . If they don't want to know what I think, they had better not ask me.

There are ways to be honest and not hurt feelings. If they ask you if you like what they are wearing say yes, but give a suggestion on a way to improve themself

Not often! My friend's a terrible driver, and everyone tells him so, but I got stuck in the car with him and he asked me to be honest- is he that bad? And I was like, "urm, no... you are getting so much better with confidence... " I didn't want to hurt his feelings, and he kind of put me on the spot!

Of course, if you know it's not what they want to hear try and put it in a nice way, but friend are there to help eachother not lie even if they may not like it, it's better to hear it from a friend than anyone else.
