The first one is correct because "lately" implies a link between present and past, and that's the "present perfect" (have slept).
The second one is completely incorrect because the tense "present" doesn't draw this link between present and past.

1 is correct becuase it states "lately", which isn't necessarily present, so I have not slept well lately sounds more correct. "I don't sleep well lately is not right because like I said "lately isn't necessarly present and "don't is used in present tense. Take a look at a couple of examples that may help show what I'm talking aobut: I don't go to the store.(Present). "I haven't went to the store" states that is something that hasn't been done and isn't exactly present.
rainbowunweaver2002,
I thought your tongue language was "Romanian" or "Italian".

Both are correct and mean approximately the same thing. There is a *slight* difference between them. The first one is more likely to be used, for one thing.
The first one emphasizes the past. The second emphasizes the present. But *both* refer to something that has been happening and is *still* happening.

One is correct, as is "I haven't been sleeping well lately." They are basically interchangeable.
Two is absolutely, positively *wrong*. "I don't sleep well" is grammatically correct, but strange because it implies there is something wrong with you, and you and fundamentally unable to have a good night's sleep.

I agree with polskajason that Two suggests that you are fundamentally unable to sleep well, but I disagree that that makes it wrong. The "lately" does suggest that it's a new development, but still, it sounds extremely defeatist.
Nothing wrong with One.

# 1 is correct
"lately" is a so-called signal word that is used together with the present perfect.
