Modern aircraft are now equipped with TCAS (or ACAS), they are intended to prevent mid-air collisions (from all sides), but they cannot prevent 100% of the chances for a midair. This was demonstrated by the accident between a DHL 757 and a Bashkirian Airlines TU-154 over Germany in July 2002, both aircraft were equipped with TCAS.

It's not like in a car, where a head-on collision is usually much more damaging than a side or rear impact due to the combined speeds. In aircraft, almost any airborne collision, regardless of direction, is likely to be fatal for both aircraft. So, it really doesn't matter whether it's a "head-on" collision; any impact is bad.

San Diego, CA. I believe in Oct.1979. I was in the Navy when it happened. A small private aircraft collided with a large passenger jet near Lindbergh Field. Why? I don't know. The event made national news and was covered by the San Diego Union newspaper. Their archives will have the details I can't give you.

yes,there was one in 1973 over france that killed 68 and one in 6/29/05 over Randolph Texas the air force base that killed 1 and critically injured anothor of course they crash in mid air thats how.
