If the tree was damaged, diseased, dying or in any other dangerous condition before it fell on to your property, then the neighbor was negligent and the claim should be filed on his insurance (assuming he has a policy). If the tree was blown on to your property by a strong wind (tornado, hurricane or even struck by lightning) and this caused it to fall on your property then the neighbor is not responsible and you should file it on your own insurance. Call your Insurance Agent and let them know what happened and they will advise you.

Your neighbor's insurance company is responsible to pay damages. However, you file the claim.

You might want to talk to you neighbor to see if you can resolve it without the insurance companies; it'll keep premiums from going up.

It's your property that gets damaged, so it would be your claim. As far as cleaning it up, that's up to you and your neighbor.
