It could be obsessive compulsive disorder, but that is a diagnosis for a professional to make!

Does he have compulsive energy behavior or just compulsive? Can he concenterate?
if he is either one add or adhd you can give him a spoon full of peanut butter or a cup of coffee and it will calm him down. I have done this with my son for years and it has worked.

Try not to put him on the meds! They do a lot of damage, to each their own but I had my son on one of the meds and the school called social services (even knowing he was on meds) that he was being starved, he lost a lot of weight.

He didn't like the way that the meds made him feel either. He was not my child! It can do a lot of inner damage to different organs too.

If you are going to put him on meds make sure you read up on them first

First thing to do would see how he does in the class room. Than if he can't focus than u could take him to a doctor that knows about ADHD. My diagnosis at 5yrs old by his kindergarten teacher. He is know 27 and have to say his life has not been easy but he doesn't quit. Please do research because a lot of these children turn to drugs to help them feel normal. It may be a phase he is going through. Some kids just need a little more attention than they are already getting, a change in their life may have that affect too. If you're really concerned about it, go see a pediatrician.

Take him to a psychiatrist and they will evaluate him. I did this for my daughter and she is bipolar. But with the correct meds he will be fine....
