Serious answer alert:
It is entirely psychological. It's the same reason why women even wear some type of gown while having stuff jammed up there. Logically you're thinking, what the hell is the gown hiding. But the fact that the patient can't really see you peering down there allows them to have a sort of a disconnect from the whole experience and therefore causes less anxiety and embarassment.

When I go to the ob/gyn the nurse enters the room first and takes my vitals. She instructs me to put on the gown and she leaves the room. I then wait for the gynecologist to come in and start the exam.

a lot of women feel uncomfortable getting undressed in front of people. when the dr leaves they're dressed and when the dr comes back they're in the gown.

If you're at a play, do you see the actresses changing on stage? No, they change out back and then pop out looking different. So that's my answer, the doc likes to be surprised.
