I know Singapore Airlines (SIA) still does. [It doesn't discriminate between classes]

Philippine airlines do, but only when you ride in first class and also the flight attendants used to get on her knees to put on those little sock like shoes on your feet, but last time i flew they didnt do it. The service is also great even if you are not flying first class.
If you ever happen to be in the Philippines fly Philippines airlines it is definitely one of the best airlines i have flown in.

bmi still gives them to everyone. Most other airlines limit it to Business and First class.

from my experience in Business class all do. That is all major carriers. I can speak for the simple no-names that barely fly around in some of the countries I've been to.
