It's probably not a good idea to call a person mental to his/her face unless your are joking around and the person can take a joke.
Sometimes we say a person is mental if they make comments that don't make any sense (like my daughter better not speak to any male teachers because I'm suspicious of all of them). Or also if they do things to harm themselves or others, for example, a parent who neglects/abuses their defenseless babies or children, but doesn't see what they are doing.
