There's a form application you can obtain from the USCIS Web site (http://uscis.gov) I believe it's form I-825, but search for it to make sure.

In my opinion, read the instructions first before getting a lawyer. Most immigration lawyers are outright ruthess crooks. The instructions maybe intimidation , but are pretty much straightforward. You can call the closest immigration office for assistance too.

I believe someone at the court house can do that for you,or a lawyer.It will cost you between $50-$100,depending where you live for the lawyer.
