some over analyze and overgeneralize ADD and ADHD, even though you might have it, you might also just need to learn how to put yourself in a situation where you can focus (IE, no TV, radio, by yourself) Also a change in your diet can help you calm down and be able to think clearly. Exercise may help too. I recommend either seeing a dietitian or a psychologist who can give you techniques to practice when these problems occur.

First I guess you don´t do drugs, in this line check a doctor not a psychiatry. Get some vitamins an do some sports.
Then in any subject you want to focus use a dictionary for each word you don't understand simple common words included these are the most important.

if you let your mind get too busy with too many thoughts it is hard to focus, try calming down and letting your mind come into focus. instead of rushing into another problem.
