Depending on the size of the ensemble:

Twin...has one pillow case/sham, one fitted sheet and one flat sheet. It may contain a bedskirt and comforter or quilt.

Double, Queen and King size ensembles contain 2 pillow cases/shams instead of one but all other co-ordinating pieces should be the same as above.

It depends on both the manufacturer and retailer. You can often purchase ensembles which include comforter/coverlet/bedspread, with pillow shams designed for the size bed you are buying for and a bedskirt. I have, however, even seen some retailers that will include a sheet set including one fitted, one flat and two pillowcases with the comforter set. I do not advise purchasing a bedding ensemble including a sheet set because the sheets will really add to the cost without providing much value. (The ones I've seen really include low quality sheets, like percale and low thread counts.)

Most Comforter sets include 2 shams (twins-1)comforter and a bed skirt. A Bed In a Bag also includes a sheet set.
