no, not all condoms work. everyone i know has had at least one experience where the condom broke. this could be for a number of reasons:
1.the condom could have been subjected to heat
2.the condom may have been carried around for a while, say in his backpocket or wallet
3.the man failed to let out all the air from the tip of the condom before putting it on
4.there was too much friction during intercourse
condoms are the most likely form of birth control, with a 99% chance of protecting you against pregnancy. the pill is good also, but will not protect you whatsoever against any STDs [sexually transmitted diseases]
you can use the following site to browse through various birth control methods available:

remember, the only way you can guaranttee you won't get sick or pregnant is through abstienence, meaning you don't have sex.

The best form of birth control is called abstinence. Try it. It is 100% successful and you never have to worry about being pregnant.

Always a chance, maybe 1 in a million, but that's all it takes. God decides, now if you are under age and worring then maybe your not ready for this.
