Grab a trash can for the spew if possible.
If no can was available, I'd sit down and hope my aim was good enough to spew into the bowl between my legs!

Sit on the loo facing it and barf into the cystern after taking off the lid.

look for a porta john at least then you can puke in the uranal and potty in the basin... :) or just find a stall and pray you dont see anyone you know.
