I think most people who learn a second language, go through this.
One thing is that you are most likely still thinking in your native language, this causes a lag time and becomes very tiring. When you are speaking the brain is 'geared up' for the second language.
The more time you can spend, in a English speaking environment (preferably with people, but can be radio or TV) the quicker you will be able to switch back an forth.
Something else to remember is that not only do you have to deal with accents and slang, but there are TWO different English languages. Queen's English (British) & American English. While the two are very similar, there are some major differences!
Don't forget, you can always ask them to speak slower. Also if you don't understand something let them know, the odds are they won't mind repeating or re-phrasing.

That is probably because they are speaking very quickly and using slang. It's hard to learn another language and frustrating when you study hard and then cannot understand someone.
If you can rent DVDs or videos that you can rewind if you can't understand the English and listen to over and over. Find someone who will tutor you by speaking slowly at first so that you can get used to the sounds.
Don't give up. You never stop learning a new language. It is a lifelong process.

the problem we all have been faced when learning a foreign language is that you are tought standard pronounciation which no one uses except on TV or radio. Any language consists of a variety of dialects which differ a lot from the standard one and people tend to speak very fast which can cause problems as well.
My advice is if you don't understand someone, interupt him and ask to repeat it or explain it to you. no one will mock you or get angry.

Actively look for people, locations and activities that will force you to learn to listen and understand. A good English speaking boy friend or girl friend also helps!

Learning from books is not enough, you must practise it by listening to English speakers accents too.
When you begin to catch the waves, you will be learning real English.
