Depends on what Grandma's doing. When in Rome.....
It might actually be better for pants off if she's senile. It will help her get a better hold of things.

My first inclination would be to keep them on. But if Grandma takes HER pants off, then all guests should remove their pants. Or better yet, the guests can remove EACH OTHER'S pants.
