The term "First Lady" is given to the wife of the chief executive of a country, state, or city.
It is also a woman who is knowledgable and educated on matters that can influence and change the behavior and opinion of people
Some women are not paid, and unelected.
Such as myself. I use my title because I believe it is a perfect way to describe my ways of helping others.

It's a title that evolved over time, A wife is typically referred to as the lady of the house. If you think of the country as one big house then the wife of the president would be the lady of that house, or the First Lady.

She's the wife of the president. Do you want to be the man that runs this country and has to go to bed each night with your wife without giving her an important title!

Well technically it's because she's the wife of the most powerful man in America, so she's the "first lady".
Does that make sense? Hope it does.
