I have never seen a video game with that rating,but ESRB lists that as a rating.Also what has to in the game for it recive that rating.

Even though there is a AO rating, there hasn't been a game that has actually been sold that has/had a AO rating.
After the Hot Coffee mod was found out in San Andreas, it was given a AO rating, but after a newer version of San Andreas was released, it was released again as a rated M game.
Manhunt 2 was given a AO rating before it came out, but Rockstar had to make adjustments to make it a rated M game because their was no major retailer that would sell the game with a AO rating.

GTA San Andreas got that rating. Also there aren't that many games with that rating
