It will depend on the rules of your state, and the time that the license has expired! If it has only expired a short time, most likely you will not need to take the test again.

Check with your state's DMV or equivalent to see what is required!

Not in the UK it doesn't,you just need a new one.

Unless you are 70,then you need a self cert that says you are fit to drive.

Yup. Most likely just the written portion, but if you contact the DMV, they'll tell ya. It varies from state to state.
