I try to change the subject to a lighter note so that they can get their mind off the awkward situation. Or sometimes the best way is to address it and make a joke out of it if no one else is around.

I'm pretty good at this; I usually don't emanate any vibe of awkwardness, so it's easy to play it like there is no awkward situation to begin with.

It depends on what you need.
If its for a health concerns, I would let you have it . Then pay me back with the change.
If its for cigarettes then no.
