What is the foundation for ? A garage, small tool shed ? The bigger the project the more reinforcement needed. Also if a big project then the city will have building codes that need to be satisfied.

You need an engineered set of drawings calculated to carry to the load for whatever you're building. After you get the drawings you'll have to submit them to the city to get their approval. Pay for the permits and begin work. The drawing will tell you what you need. You'll most likely have to dig, build forms, add rebar, and possibly tie the rebar into any exiting structure by doweling and using epoxy. You'll probably need anchor bolts set as well. After all that you'd call for inspection before you pour.
You may need a pumper to pump the concrete from the concrete truck if you can't get the truck to where you need it, or you would have to use wheel barrows and shovels. Fill the forms and screed with a 2x4 or something. If the appearance is not critical finish with a wood float. strip the forms the following day.
Or just pay a contractor to do all this back breaking work for you.
