I've heard it could be a venting problem. I removed the clean out cap to main linejust to see if it would flush better and toilet flushed fine. The vent line runs a span of 15' or so under house. It does have sag in middle of pipe, could water get trapped in there causing less air flow? Should there be more than one vent pipe?

yes on the vent line, , if you can wire the sag part up, to make line level,
no only one vent is needed,

yes water trapped there can restrict the air flow causing the toilet to not work properly. I would suggest fastening up the pipe but you really need to have a way to drain the pipe when needed as well. at the lowest part of the pipe i would cut it and splice in a drain valve. that way if need be you can drain the pipe easier , later.

every trap needs a vent, but it is probably the sag that is causing the problem... try jacking that up so there is no sag and see if that helps.

you should only need 1 vent pipe per toilet.. if it continues you may need a plumber to come snake the pipe out..
