i have not eaten in every place in the world but i must say the one that sounds most terrible has to be bathroom using the toilet seat as a table....

Kolkata, India, a little hole in the wall place called Peter Cat. It looks promising from the outside, but once inside you realize everything on the table is just kind of recycled. Water is brown or amber, what is cold is lukewarm, what is hot is cold, and what should not give you parasites does. Sad times

For me, the worst place for food and service ( also the decor )
would have to be a hospital Cafe.

I think somewhere like China, China, you know those chinese eat all you can buffet style restaurants, YEUCH! I much prefer quality as opposed to quantity.
