as far i am aware nobody fell out of x-scream, that would have made worldwide headlines, there was the problem when a group were left hanging over the edge for about 90 minutes back in 2005 http://kvbc.com/Global/story.asp?s=4130429

First, the coaster is gone. No one ever fell out. One of the other rides stuck a couple of times but NO one was every hurt.

My dad works at the Stratosphere Hotel and Casino and he said that this guy paid to ride a ride then he just jumped out and died! He also said that this wasn't on the news.
