A rain shadow area is the leeward side of a mountain which blocks the moisture laden winds and make shed all the moiture as rain on the windward side of the mountain thereby denying the rainfall on the leeward side.For example,the eastern side of the Western Ghats
in the peninsular India is a rain shadow area as it blocks the south-west monsoon winds resulting in heavy rain on the western side whereas the eastern side gets very little rain.
Heat islands usually exist in the cities.Cities consume enormous amounts of energy in heating,cooling and transportation.The energy is released as heat.This heat is trapped under a layer of pollution(as pollution is more over cities) that arches over the city and prevents from escaping.Added to this,building materials and road surfaces store heat.So, cities are typically several degrees warmer than the surrounding rural land.So, cities become island of heat.
So,rain shadow area and heat islands are two different things ;they cannot be compared.One is a rainless area and another is the hottest area.

A rain shadow is the area behind a mountain where the mountain prevents rain from falling, usually by elevating the air and wringing the moisture on the other side.
A heat island is usually a city where the landscape results in hot air being created and rising.
