If you format your memory stick on the PSP , it will erase all the games/music/movies and pictures on it but keep the layout so the memory card will still work with the PSP , If you format it on the computer , everything will be erased so it will no longer work with the psp

That depends. If you format it from your computer, it will erase everything, including the folders (music, pictures, movies, etc.)
If you format from your PSP menu, however, it will delete everything except those folders. It will, however, delete whatever is in those folders. This is done so the PSP can recognize what's on the stick. I don't think you have to format the stick it came with (if it did), because mine came formated out of the box.

When you format memory, it is prepared to receive data from the source. Any data in the memory will be lost forever once you format it.
