Well the Grunge movement originated in the Seattle, Washington Inspired by hardcore punk, heavy metal and indie rock, grunge is generally characterized by heavily distorted electric guitars, contrasting song dynamics, and apathetic or angst-filled lyrics. The grunge aesthetic is stripped-down compared to other forms of rock music, and many grunge musicians were noted for their unkempt appearances and rejection of theatrics.
Mainstream Rock such as GNR is all about theatrics, and Sex Drugs and Rock & Roll GNR would actually be more considered Glam/Hair Metal than Grunge.Generally, glam metal has hedonistic lyrics, often focuses on sex, alcohol, and drugs, and many of the first wave bands had songs pertaining to the occult. Musically, glam metal songs often featured distorted guitar riffs, shred guitar solos, anthemic choruses, hard hitting drumming, and complementary bass.

The only difference is the sound. Nirvana fans had a perception that the music they listened to was not mainstream, but it was as mainstream as you could get.
