It depends on where in the sentence it falls.
He and I ate lunch.
The book belonged to him and me.
When you are writing the sentence think about which you would use separately.
He ate lunch. I ate lunch. (so He and I)
The book belonged to him. The book belonged to me. (so him and me)

It depends on the situation. It would be
he and i work at the bakery
The gift is from him and me.
The way you figure it out is that you always put yourself last (which you seem to know already) and you ask
If I said it is from (he/him) would it be right?
If I said it is from (I/me), which would be right.
The two answers you give are the ones that you say, with the conjunction me.
You can also insert commas for more people.
