According to Tom Bakers Doctor (4th doctor) he is a Doctor of everything. In this case the designation Doctor is educational not just medical. After 900 odd years he has had plenty of time to figure out most of life's mysteries. In the same vein is his adversary, the Master. Both names being types of degrees one can get from a University. Master being the lesser degree (of course).
