Some people don't see the parallels in the situations they deal with. They are unable to transfer the lessons in one stuation to the next. Some people don't even think about what they are going through and what they must do differently. It doesn't just take that you don't want to do it, it takes a willingness and the action of doing something different. Many people don't want to go through the effort of doing things different.

I think people make the same mistake because even if they do realize it, they jus don't know how to better go about it. They haven't been exposed to a better way. So they keep doing the same mistake over and over again due to a lack of knowledge and experience.
Eventually they will learn from life, or something will hit them. At least I hope so.

Because they either don't remember the previous mistake at the time, or because they don't recognize (or are in denial) of the similarity between the current and past situations.
Basically, we engage our mouth before our brain, or we do a thing-AGAIN-without thinking. Much of our behavior is on "automatic," i.e. we just aren't thinking, or we are thinking about something else. Let's see, how many times have I locked my key in my car...or overdrafted my checking account just because I didn't check the balance.
And often, even in important matters, like relationships, we want something so intensely that we deny the similarity between this situation (relationship) and the previous one.
Also, much of our behavior is composed of habits, which are automatic behaviors. Again, we are not thinking of the previous behavior when we repeat our mistake.
