It could be due cuz of the constant rubbing of his t shirt.
That can cause skin irritation,
Try putting dipper rash cream and some band aids so the cream won’t rub off...
If that doesn’t work...

If it doesn't go away within a day or two he should see a doctor. There could be a cyst or something behind it.
