Well it sounds like a paracite or disease

If you have, or can get melafix this sorts out a lot of things and works well with bettas

If not, then adding one teaspoon of aquarium salt per gallon of water in the tank might help

He is quite old, although bettas can live for around 10 years old

How big is his tank?
How active is the fish?
Is the tank kept at a constant temperature?
Have you checked ammonia/nitrite and ph levels?

There are many things that effect the health of fish

ADD

Im glad he is in a 10 gallon tank with a filter, but you shouldnt turn it off. The tank with my females in (when not trying to breed them) is a 10 gallon and they always hang around in the current of the filter. If he is swimming away try turning it down or moving it so it doesnt have as much of a current. He really does need a heater and there is no reason why you couldnt add a few other fish in like tetras or danios with him if you wanted. The snail wouldnt be a problem at all. Some plants would be a good idea if you havent got any as well

That's a good size tank, well done for not putting him in a bowl!

Does he have a filter? And have you been keeping up with water changes? That's the first port of call.

Also, get him a heater. They are tropical fish, and small heaters aren't very expensive. They can live without one, but thrive with one.

The snail shouldn't bother him, so don't worry about that.

My advice:

Do a 30% water change, and the same tomorrow. Get a filter (if you don't have one) and a heater too. For the next 4 weeks, do 2-3 30% water changes per week. He should be much happier.

The other option is if it's an illness. In that case, I recommend melafix.

Good luck!

Does he look like someone sprinkled him with a salt shaker? He most likely is suffering from ich. The best treatment would be to add aquarium salt ( get the Dr. Wellfish kind- can find it at Walmart , petsmart etc) and disolve it according to package directions. I ussually disolve it in a little container of tank water and then add that to the tank. Also you do need the heater if it is ich. If you get a heater make sure to also get a thermometer to keep track of the temp. for ich I would raise the temp to around 82 and keep it there for a couple weeks along with the salt treatment. Salt and heat will be affective in killing the ich. good site for info is www.uniquebettas.com

You can get stuff for it at any pet store or where ever they sell fish. I'm not sure what it's called, but it's pretty cheap. Ask a store rep or look in the fish section.
