When someone prefaces their remark with "No offense," be prepared to take offense.
Just like when they say, "Not to change the subject, or anything," that's exactly what they're going to do.
Oh, yeah, and "Far be it from me to judge," means that they are about to tell you why someone (possibly you) is, in their opinion, going straight to hell.
I could go on all night, but those are the three that really get under my skin.

I don't know.
Maybe "lol". It's one of the most commonly used phrases yet no one is ever really actually "laughing out loud" so basically, it's a pile of crap.
