No why would I be offended. I mean I am a female which is a lady so no I have never been insulted when someone in a restaurant or not has called me a lady. Why? I want to here a story if there is one.

That depends on the context of how it is used. if the waiter says how are you ladies today? Or what would you ladies like to order today ? or something of that nature it would be okay,as long as their are 2 or more "ladies " in the party. But if the waiter would say lady what are you going to have today? No! Or what are you having today young lady? again No!!!! Or Hey lady! No!!!!Another thing I can't stand is for a server to cal me "Honey" or "Dear" ,"Dearie" Any of those things are very disrespectful.

Heck no.
As a server (in my 30's) I would call ladies younger than me ma'am (or miss on occasion). It is just a show of politeness and is not meant to insinuate age or anything else.
As with all terms or titles, vocal insinuation and attitude can play a huge factor, but I would wager that the majority of times "lady" or "ma'am" are used, they are meant in a polite way.

Yeah if there are a set of ladies call them females or women and if it quite is women and adult males in simple terms call them adult males like "could you adult males decide to start up with some appetizers this night?". it quite is often what waiters do at my table whilst it quite is me, my mom and my dad.

actually, restaurants (at least fine dining restaurants) train their employees to address females as "miss" or "lady" depending on their age. "guys" is considered inappropriate and most women who feel younger than they are are offended by "mam."

It depends on how they say it. If they're like "And for the lady?" that is totally polite and normal.
But if they're like "Excuse me lady," or "Hey lady!" that is rude. ;o)
If you have to get a person's attention, it is better to say "Excuse me miss," unless they're really old.

It would depend how they say it...like "lady" or "laday" or ladeeee all ghetto like or i guess i would have to be there. "Miss" sounds better.
