I would just request a leave of absence for a month due to family medical problems. If you feel that you need to elaborate more on that then do so, but they really don't need to know your personal problems.

Are you in the US? You might be covered by the FMLA (Family and Medical Leave Act) if you have been at your job for a year, and they have over 50 employees. If you are covered under FMLA, they are required to give you the leave. They don't have to pay you for it, but your job is protected. If you think you might be covered and have an HR department where you work, see them for the proper paperwork.
If FMLA isn't available for you, you'd probably be better off sitting down with your manager and telling him or her about the situation, then following up with a short letter saying something like "Confirming our discussion today, I am requesting a leave of absence for one month, starting xxxxxx."

ask for some time off due to illness in the family. Ask your HR person if there is a certain form or its a free form, make sure your manager he/she ok's it and gives a month of so off. Hope you dad gets better.

Enlist the help of the ofc mgr either by phone or personal visit. They may have a disability plan than may help you get at least partial pay if not all. It's his job to assist you.

Just tell the truth in your words. If the letter is too eloquent then this boss will be suspicious and ask you questions like, "How do you know you have to be with him for a month?"
